PARENTAL CONSENT AND TRANSPORTATION RELEASE

I, the undersigned parent or legal guardian of the child(ren) registered above, give permission for my child(ren) to participate in activities sponsored by Trinity Baptist Church, including transportation to and from church services and events.

I understand that reasonable precautions will be taken to ensure the safety of all participants; however, I acknowledge that transportation and participation in activities involve inherent risks.

I hereby release and hold harmless Trinity Baptist Church, its staff, volunteers, and drivers from any liability for injuries or damages arising from participation in these activities, except in cases of gross negligence or willful misconduct.

In the event of a medical emergency, I authorize the church and its representatives to obtain necessary medical treatment for my child.

I agree that I or my family's representative will be home to receive my child(ren) at the time agreed so as to not allow my child to be left alone or delay the transportation of other children.

By signing this form, I confirm that I have read and understood the terms outlined above.

I freely and voluntarily give my consent to participate in the activity, program, or service described. I understand the responsibilities, requirements, and any potential risks involved.

I accept the terms and conditions of this agreement and agree to comply with all related rules and policies. I understand that I may withdraw my consent at any time by contacting the organization, recognizing that doing so may affect my and my child(ren)'s ability to continue with the related service or activity.
